To order this product on Costco.com, please enter the following information for the “Shipping Address” at checkout:

First and last name:  name of the deceased
Company name:  name of the funeral home
Address:  address of funeral home (or location where casket will be delivered)
Phone number:  phone number of the funeral home

For the “Billing Address," please enter the following information at checkout about the person or organization that is paying for the casket:

First and last name: name of the bill-to customer
Company name:  name of the bill-to company name (if applicable)
Address:  address of the bill-to customer
Phone number:  phone number of bill-to customer

Prime Casket Company only makes deliveries to funeral homes and cemeteries.  Deliveries to residences or commercial locations are not permitted.

Eligible States: Currently, caskets can only be purchased from and shipped to addresses in the following states:
